Listening and responding to patients’ stories for over 20 years as an emergency physician has strengthened my appreciation for the many ways that the skills and principles drawn from writing fiction double as necessary clinical skills. The best medicine doesn’t work on the wrong story, and the stories patients tell sometimes feel like first drafts—vital and fragile works-in-progress. Increasingly complex health challenges compounded by social, financial, and psychological burdens make for stories that are difficult to articulate and comprehend. In this (...) essay, I argue that healthcare providers need to think like creative writers and the skills and sensitivities necessary to story construction deserve a vital space in medical education. A thorough understanding of story anatomy and the imaginative flexibility to work stories into open spaces serve as antidotes to the reductive nature of clinical decision making and have implications as patient safety and risk management strategies. The examples that I have selected demonstrate how thinking like a creative writer functions at the bedside, providing tools for clinical excellence and empathy. This approach asks that we re-imagine the importance of story in clinical care: from a vehicle to a diagnosis to its place as a critical destination. (shrink)

“I'm not afraid of dying,” he says, despite his plea on arrival. “Listen up, douchebag. Are you calling my cousin or what?” The emergency department might be the only sphere of human exchange where one party—patients (and sometimes family)—are permitted to insult, threaten, and even spit at the very people on whom they depend for help, while the offended parties—physicians, nurses, and other health care providers—must not only tolerate the abuse, but treat their tormentors. Does the ED's collective duty to (...) greater numbers of patients demand a revised ethos of tough love for extreme cases of misbehavior? Can we ask these patients to leave without legal recourse after extending genuine, compassionate efforts to participate in their care—barring evidence of a medical explanation for their toxic comportment or a mental illness that puts them at risk of harming themselves or others? BACKGROUND: Many published accounts of clinical trials report no differences between the treatment arms, while being underpowered to find differences. This study determined how the authors of these reports interpreted their findings. STUDY DESIGN: We examined 54 reports of surgical trials chosen randomly from a database of 110 influential trials conducted in 2008. Seven that reported having adequate statistical power were excluded from further analysis, as were the 32 that reported significant differences between the treatment arms. We examined the remaining (...) 15 to see whether the authors interpreted their negative findings appropriately. Appropriate interpretations discussed the lack of power and/or called for larger studies. RESULTS: Three of the 7 trials that did not report an a priori power calculation offered inappropriate interpretations, as did 3 of the 8 trials that reported an a priori power < 0.90. However, we examined only a modest number of trial reports from 1 year. CONCLUSIONS: Negative findings in underpowered trials were often interpreted as showing the equivalence of the treatment arms with no discussion of the issue of being underpowered. This may lead clinicians to accept new treatments that have not been validated. (shrink)

Most behavioral decision research takes place in carefully controlled laboratory settings, and examination of relationships between performance and specific real-world decision outcomes is rare. One prior study shows that people who perform better on hypothetical decision tasks, assessed using the Adult Decision-Making Competence (A-DMC) measure, also tend to experience better real-world decision outcomes, as reported on the Decision Outcomes Inventory (DOI). The DOI score reflects avoidance of outcomes that could result from poor decisions, ranging from serious (e.g., bankruptcy) to minor (...) (e.g., blisters from sunburn). The present analyses go beyond the initial work, which focused on the overall DOI score, by analyzing the relationships between specific decision outcomes and A-DMC performance. Most outcomes are significantly more likely among people with lower A-DMC scores, even after taking into account two variables expected to produce worse real-world decision outcomes: younger age and lower socio-economic status. We discuss the usefulness of DOI as a measure of successful real-world decision making. (shrink)

There has been a growing concern over establishing norms that ensure the ethically acceptable and scientifically sound conduct of clinical trials. Among the leading norms internationally are the World Medical Association's Declaration of Helsinki, guidelines by the Council for International Organizations of Medical Sciences, the International Conference on Harmonization's standards for industry, and the CONSORT group's reporting norms, in addition to the influential U.S. Federal Common Rule, Food and Drug Administration's body of regulations, and information sheets by the Department of (...) Health and Human Services. There are also many norms published at more local levels by official agencies and professional groups.Any account of international standards should cover both scientific and ethical norms at once – the two are conceptually intertwined. I’m avoiding Mr. G’s room. I shouldn’t have read the emergency room notes from the other hospital, where this middle‐aged man raised a stink about the wait. What type of person calls the triage nurse a bitch? From the timestamps on the electronic medical record notes, he stormed from that ER and drove his abdominal pain, vomiting, diarrhea, and discontent directly across town to us.I’m reminded of this oft‐quoted aphorism from Sir William Osler: “The good physician treats the disease; the (...) great physician treats the patient who has the disease.” It’s cited by medical educators as an invocation to never forget the humanistic heart at the center of clinical care. But what if the patient is a jerk? That I haven’t met Mr. G, only read briefly about him, doesn’t stop these ugly feelings from surfacing. (shrink)

But it was 3:00 a.m. and the ER was a carnival of disease and discontent, a parade of drunk drivers and folks who practiced conflict resolution with knives and bullets. Background: It is not known how neonatologists address the affective and cognitive loads on parents deciding whether to resuscitate infants born extremely preterm. This study explores expert neonatologists' views on these decision-making processes and their own roles in counseling parents. Methods: Semistructured interviews asked internationally recognized experts to share their perspectives on perinatal consultations. Their responses were subjected to thematic analysis. Results: Eighteen of 22 invited experts participated. Approximately equal numbers reported employing a physician-driven approach, a parent-driven approach, and a (...) combined approach during these consultations. Those who followed a physician-driven approach typically focused on conveying standard information about adverse outcomes. Those who followed a parent-driven approach typically focused on addressing parents' information requests, guiding their decision making, and providing affective support. Nearly all experts, in each group, endorsed addressing the child's quality of life, in terms of functionality, when discussing long-term outcomes. Although many believed that families adjusted to life with a disabled child, few discussed the topic during prenatal consultations. Most, in each group, reported trying to alleviate future “decisional regret” for parents whose premature infants subsequently became disabled. None spoke to parents about possible decisional regret after deciding to forgo resuscitation. Conclusions: Expert neonatologists are deeply concerned that parents understand the decision facing them. However, they differ on what information they offer and how they balance parents' need for cognitive and affective support. They expressed more concern about parents' decisional regret should their child survive resuscitation, but have severe disability, than about decisional regret after foregoing resuscitation. As one commentator notes, Spinoza’s conception of “the third kind of knowledge”—intuition, has been “regarded as exceptionally obscure. Some writers regard it as a kind of mystic vision; others regard it as simply unintelligible.” For Spinoza, the first kind of knowledge, which he calls “imagination,” is a kind of sense-experience of particulars; the second kind, which he calls “understanding,” involves the rational grasp of universals, and the third, in his words, “proceeds from an adequate idea of the formal essence of (...) some of the attributes of God to an adequate knowledge of the essence of things.” In this essay I will attempt to show, through an explication of Spinoza’s concept of intuition, how a prime example of intuition can be found in the art of poetry. More specifically, I will examine resonances between the work of the poet Gerard Manley Hopkins (1844–1889) and Baruch de Spinoza (1632–1677) by exploring the ways in which Hopkins’ poetry can work as both (1) an exemplar of poetry qua Spinozistic intuition, and (2) an intuition-based access to Spinoza’s thought. The upshot of this essay, then, is that there is a kind of knowledge, and by implication a kind of education (through which to acquire that knowledge) which—even for a philosopher as mathematically rigorous as Spinoza—may require recourse to the art of poetry. Objects are inert, passive, devoid of will, and as such bear no intrinsic value or moral worth. This claim is supported by the argument that to be considered a moral agent one must have a conscious will and be sufficiently free to act in accordance with that will. Since material objects, it is assumed, have no active will nor freedom, they should not be considered moral agents nor bearers of intrinsic ethical vale. Thus, the apparent “moral neutrality” of objects rests (...) upon a kind of subject/object or mind/body dualism. The aim of this paper is to explore two paths by which western thought can escape this dualism, re-valuate the alleged “moral neutrality” of material objects, and initiate a sort of “object oriented ethics,” albeit with surprising results. To do so, this paper explores the work of Arthur Schopenhauer and Baruch Spinoza to interrogate both the claim that material objects have no will and that freedom is the necessary condition for ethical responsibility. This paper concludes by arguing that not only should objects been seen as bearers of their own ethical value, a determinate judgement can be made regarding that value through a basic understanding of the laws of physics. The diverse and paradoxical nature of globalization processes has given rise to new social constellations that shape transnational, national and local spaces. The historicity of identities, their past and present conditions, the changes they went through, the ways they influence the feeling of full membership in a community and the differentiation derived from cultural diversity and pluralism underscore the need for revisiting theoretical explorations. This paper addresses past and present social, cultural and religious processes in an era of transformations derived (...) from the complexity of today’s interconnected world and on the light of historical encounters. The need for revising the singularity of social and cultural trajectories and the religious trends gravitating in society is approached through snapshots of a twofold historical encounter: between Modernity and Latin America, and between Judaism and Modernity. Both express entrenched dilemmas of the binaries periphery-center and universal-particular. While one of them raised the issue of the dominant program of Modernity as a Western project, the other was entailed in the assumptions of one hegemonic religious constellation. (shrink)

Shelford Sometime after 1679, Pierre-Daniel Huet (1630-1721) indulged an author's vanity by comparing his Demonstratio evangelica with works whose authors are far better known today. He recorded his judgments on a scrap of paper. 1First, he contrasted the Demonstratio to Antoine Arnauld's Les nouveaux élémens de (...) géometrie (1664). While Arnauld had assumed that all the principles of geometry were true and all geometrical demonstrations certain, and had considered religious questions not amenable to methodical demonstration, Huet noted, "I made clear the contrary by proving religion through a methodical sequence of propositions similar to those one finds in geometry." For Arnauld religious belief fell into the class of things better proven through a series of mutually reinforcing propositions than through proofs modeled on geometry; Huet had actually accomplished the latter. Arnauld did not require geometric proofs in religious works; Huet wrote the Demonstratio "to satisfy those who do." Indeed, Arnauld forbade such proofs, "but I permitted them... and I proposed something to satisfy those who do [demand them]."Huet then considered Blaise Pascal's Pensées (1670). Pascal had also rejected demonstration in proving religious truths, Huet noted, opting instead for "moral proofs, which are directed more to the heart than to the mind. In other words he desired to work harder at moving and disposing the heart than at convincing and persuading the intellect." Apparently, "M. Pascal did not believe that one could prove the truth of the Christian religion by geometric proofs; much less that he had any hope of doing so." [End Page 599]Huet's jottings precisely situate the Demonstratio evangelica (1679)between the claims of Cartesian rationalism and Pascal's fideism and religion of the heart, inviting us to take another look at an imperfectly understood work. Scholarly treatment of the Demonstratio has generally been cursory or partial because it has been considered less for its own merits than for how it fits into larger research agendas. 2 Some scholars have offered important insights but either do not develop them or do not consider the Demonstratio in a sufficiently broad context. 3 A consensus on the Demonstratio's general thrust thus masks puzzling questions. Everyone acknowledges, for example, that Huet's apologetic responded to Baruch Spinoza's Tractatus theologico-politicus (1670). Like other Christian apologists, he had to maintain "the Mosaic authorship" that was "the supposed guarantee of the truth of the text," 4 devoting "long chapters to the authenticity of the books of the Old Testament." 5 Yet Huet began the Demonstratio with an assault on geometrical knowledge and an epistemological discussion—an odd place to begin refuting the Tractatus. 6 Why? The question deserves a good answer, as Huet was a scholar of international reputation, and his contemporaries judged the Demonstratio an important work. 7More important for us, the evolution of the Demonstratio superbly reflects a vanished intellectual world and scholarly culture and in fact records change [End Page 600] in that world over four decades. Its genesis was the rash promise of a young man inspired by the possibilities of geometry and aspiring to recognition in the Republic of Letters, but before fulfilling that promise Huet developed a lively interest in such diverse fields as biblical criticism and natural philosophy, and he would apply the insights gained there in his apologetic. 8 A central question confronting Huet and his contemporaries was how to achieve certainty in matters religious, historical, and scientific. 9 The Demonstratio reveals Huet's conclusion that certainty in all three was profoundly interrelated. There has been much first-rate work done in recent years both by way of criticizing Humean assumptions and explicating alter native concepts of causal explanation and non-logical necessity. Roderick Chisholm early showed the inadequacies of neo-Humean views of explanation in his articles on counterfactual inference, and C. J. Ducasse, Sterling Lamprecht, William Kneale, Nicholas Maxwell, Richard Taylor, G. E. M. Anscombe, P. T. Geach, Milton Fisk, Baruch Brody, Peter Alexander, R. Harré, and William Wallace, among others, have articulated interesting (...) alternative views to the Humean ones they have so trenchantly criticized. We will be concerned with the recent work of Harré and Wallace in this study. Harré has not only launched a fresh attack but has also presented what is probably the most fully developed neorealistic [[sic]] alternative. So while he could have one less finger, he could not be other than human. These intuitions suggest that (...) there are two kinds of properties: on the one hand, there are essential properties and, on the other, there are accidental properties. Essentialism is the view that captures those intuitions and claims that some of an object’s properties are essential and some are accidental. Essential properties are often appealed to in metaphysical arguments. Consider the famous Goliath–Lumpl example. Goliath is a clay‐statue, and Lumpl is the piece of clay from which Goliath is made. Are Goliath and Lumpl the same entity, or are they distinct, despite coinciding in the same spatial region (at least for a period of time)? An argument in favour of their distinctness appeals to essential properties. Let s be Goliath’s shape. Lumpl’s current shape is therefore also s. It seems that Lumpl could lose its current shape and still exist. Yet, if Goliath – the statue – were melted, for instance, it would cease to exist. Behind this reasoning is the belief that having shape s is essential to Goliath but not to Lumpl. Goliath and Lumpl can therefore be distinguished by their essential properties. Consequently, and by Leibniz’s Indiscernibility of Identicals– according to which identicals cannot be distinguished – Goliath and Lumpl are distinct objects. The essentialist intuition appealed to in this argument is that shapes are essential to statues but not to pieces of clay. Of course, this particular argument – like any other – can be resisted. It is all the same a good illustration of how essentialism is put to work in metaphysics. A good understanding of the broad phenomenon of essentialism is therefore an unavoidable task, since the credit of a lot of metaphysical arguments partially depends on it. With this book – which is, among other things, a defence of a Direct Theory of Reference – Kripke rescues, after Quine’s powerful attacks against it, and using the semantic rigidity of proper names, (Aristotelian) essentialism. Mackie, Penelope. How Things Might Have Been: Individuals, Kinds and Essential Properties. Oxford: Clarendon Press, 2006. An extremely valuable recent contribution to the field. Here, Mackie defends that, although essentialism is a real phenomenon, there are much fewer essential properties than is standardly believed, and labels this view ‘Minimalist Essentialism’. Beyond the intrinsic interest of the defence of Minimalist Essentialism, the book is also highly recommended for its clear reconstructions – and discussions – of some of the most influential essentialist arguments. In addition, the book’s bibliography is very comprehensive and updated, and it might be a useful resource for students. Plantinga, Alvin. The Nature of Necessity. Oxford: OUP, 1974. Yet another foundational work. Here, Plantinga offers his essentialist theory which, like Forbes’, also incorporates individual essences although of a very different sort from Forbes’. Plantinga believes in purely non‐qualitative individual essences, whereas Forbes’ aims at purely qualitative ones. The book is also excellent to provide a solid background to those unfamiliar with the topic. Roca‐Royes. ‘Essentialism vis‐à‐vis Possibilia, Modal Logic and Necessitism.’Philosophy Compass, 6/1 (2011): 54–64, doi: 10.1111/j.1747‐9991.2010.00363.x. This is one of the two papers this guide accompanies. It contains a comparative examination of two opposed responses to the problem of possibilia. Against what Kripke’s Naming and Necessity might have induced us to believe, Salmon’s Reference and Essence argues that essentialism cannot be derived from the Direct Theory of Reference. Our commitments to essentialism should therefore come from somewhere else. Sample Syllabus Because the phenomenon of essentialism is very broad, one can design an entire course on it, thereby having enough time to see some of the issues in considerable depth. The sample syllabus suggested here is of one such course, focusing exclusively on contemporary issues in essentialism. Focus Questions • There is a debate as to whether essential properties– as well as individual essences and sufficient‐for‐existence properties– should be specified in purely qualitative terms or whether, by contrast, non‐purely qualitative – or even purely non‐qualitative – specifications are allowed. What are the main arguments against the availability of purely qualitative specifications? • Flexible essential properties seem to be more plausible than non‐flexible ones. Yet, together with sufficient‐for‐existence properties, they imply flexible individual essences. How plausible are these? Should we revise our intuitions supporting flexible essential properties? • What are, ultimately, the ontological differences between a necessitist approach to possibilia and an individual‐essences‐based approach? • According to necessitism, essential properties do not impose necessary conditions for an object’s existence. Can the necessitist, nonetheless, capture this role of essential properties by saying that, when it comes to possibly concrete entities, essential properties provide necessary conditions for an object’s concreteness? Can they, on a similar vein, also offer sufficient‐for‐concreteness properties as a surrogate for the (non‐necessitist’s) sufficient‐for‐existence properties? • Even after the enormous amount of literature that essentialism has generated, we are far from reaching a general agreement as to which properties are essential and which are accidental. It seems to me that those who place great value on the right to human freedom can be badly divided on the question of the use of force by states to defend the liberties of those who are not citizens of that particular state. Concerned about the liberties to be defended, they might be enthusiastic supporters of the use of such force by liberty-loving countries throughout the world. Concerned about the liberties that might be violated when the state marshals its (...) forces for use internationally, they might adopt a more isolationist approach to this issue. This paper is an attempt to help clarify this conflict by looking at some of the philosophical issues it raises. Because I wish to avoid factual debates about current conflicts, I will give no real-life examples. However, they are on my mind, and I hope the reader will keep them in mind as well. (shrink)
